If you have been injured in an accident, let me start off by saying that you have my sincere sympathy and warmest well wishes. I would also like to congratulate you for looking for a way to take charge of your life!
I am sure that no matter what type of accident or injury you have been involved in (e.g. car, work, slip and fall, attack), your life has been turned upside down. And I wouldn't be surprised if you are not receiving the support you should be from those who are supposed to be helping you.
You see, in the many years I have worked in the accident injury arena, I have witnessed how insurance companies and doctors so often fail to support the patient. Not all doctors and insurance companies are bad. But, as you may know, there are many that are questionable. One of the key points I have learned over the years is: "If it is not documented, it was never done"! Hence, the Accident Documentation Kit was produced.
Welcome to my website. My name is Dr. Michael J. Kaye, and I am the author of the Accident Documentation Kit. As the director of The Rehabilitation Group of Bucks/Montgomery County (a clinic with an emphasis on treating chronic pain and bringing about wellness), I originally developed this kit for our patients.

You have so many things to deal with already that your life probably seems to have been put on hold. The main focus of your day may soon become, if it hasn't already, interaction with doctors, insurance companies, attorneys, and perhaps your employer. It is very important to keep detailed written records of these interactions and of everything pertaining to your accident.
You MUST document all of your appointments; communications with your employer, doctors, and insurance company; the activities of your life since you have been injured; and all expenses related to your recovery. This information will be necessary for insurance claims and if you have to defend yourself to your employer or in court.
I can't stress enough – one of the key points I have learned over the years:
"If it is not documented, it was never done"!
I do not mean to imply that most doctors are unethical (many, as myself, have impeccable values and truly care about their patients), but it doesn't pay to hide our heads in the sand.
Here are a few examples of things I've learned that I will tell you about in the kit, regarding how the system works and why it is so important to document and to choose a good health team (which is another process my kit will help you navigate).
FACT – Having insurance is truly a blessing. But dealing with insurance companies is often a HUGE HASSLE. They may misplace your "Explanation of Benefits" or tell you they can't find you in their system. Again, you must keep good records. If you cannot back up your claims, the insurance companies might use any excuse to pay you the least that they can.
FACT – In the worker’s comp and personal injury system, the insurance company has the right by law to send you for an Independent Medical Examination (IME). The insurance company designates a doctor to perform an examination, and the doctor receives payment from the company in the range of $150 to $500. It would appear that the IME doctor has a financial incentive to report to the insurance company that care is no longer needed, just as the company has a financial incentive to deny further care. Scary and sad, but true.

Being a do-it-yourself kind of person, I have bought packages of forms before from various sources, such as paralegal stores and discount online venues.
